Transport Consent Form

This form is to be completed by parents/carers who require transport services from Walsall Childminding Hub. By completing this form, you are providing consent for your child to be transported by authorised, vetted and suitably qualified staff members for childcare related activities.

We understand that your child’s safety and wellbeing during travel is extremely important. This form enables us to obtain parental/carer consent for the transportation of children for purposes including school or nursery collections, transport to and from the setting, outings, trips and emergency situations where transport may be necessary.

All staff members responsible for transporting children will have appropriate booster seats or child restraints available in their vehicles, suitable for each child’s age, height, and weight. Every child will be safely secured before travel in accordance with current road safety regulations and best practice guidance.

In addition, all staff members providing transport will hold an OFSTED suitability letter, a valid DBS and a current Paediatric First Aid certificate. A fully stocked first aid kit will also be kept in the vehicle at all times to ensure staff are prepared to respond appropriately in the event of an emergency.

Parents/carers have the right to request to see these documents at any time. This can be arranged during a face-to-face meeting or at the time of collection or drop-off.
